If you choose to book an appointment with Reynolds Chiropractic, we have the Patient Intake Forms here for you to download to save you some time. If you prefer to complete the intake at the office, that is fine also. You may need to arrive about 15 min’s prior to appointment.

The first appointment may take an hour. Again, Dr. Reynolds listens and wants to make sure this is a good fit for you. We are looking forward to meeting you and helping you get back to the life and activities you enjoy!
